Pre-Paid Packages
- $450 for 6 visits ($75 per visit)
- $650 for 10 visits ($65 per visit)
- Patients are eligible to purchase packages only after going through the initial visit.
Pre-Paid Package Policies
- The Treatment Package is a program where a discount is applied toward prepaid visits.
- Visits are non-transferable to other patients.
- If you wish to cancel the package before using up all the visits, you must notify us in writing (email preferred).
- The refund amount will be calculated by subtracting the cost of each used visit (at regular, non-discounted rates of $85) from the original paid amount. Refunds will be given within 1 week of the written request.
For example:
Refund calculation for cancellation of $450 package after using 2 visits:
$450 – (2 x $85) = $280 total refund
- One visit will be subtracted from the prepaid visits for any no-shows or cancellations within 24 hours of your appointment.
Expiration of Package Values
- Your package technically does not expire, however, the discounted value expires 1 year after purchase.
- After 1 year, you will have a credit on file minus the amount of used visits.
- The remaining amount can be used toward future purchases.
- Refund policies (above) still apply should you request one.
For example:
$450 6 visit package is purchased on Jan 1 ($75 per visit). 2 visits are used in that calendar year. Jan 1 of the following year, the discount is considered expired. The unused amount is then applied as an account credit.
$450 – (2 x $75) = $300 credit
The credit amount can then be used toward full rate single visits or toward a new package.
